For mye og for lite. Oksygen til ekstremt premature nyfødte.
Abstract
Bakgrunn: Oksygen betraktes som et medikament og kan ha flere bivirkninger. Både for mye og for lite oksygen kan ha konsekvenser for barnets fremtid. Nyfødtsykepleieren skal forebygge at barnet påføres kortsiktige og langsiktige komplikasjoner av behandlingen.
Problemstillingen i denne oppgaven er: Hvordan kan nyfødtsykepleieren forebygge hypoksi og hyperoksi hos det ekstremt premature barnet?
Metode: Oppgaven er en litteraturstudie og metoden er å gjennomsøke litterære kilder. Systematiske litteratursøk er gjort i følgende elektroniske databaser: Medline, Cinahl, Embase og Cochrane Library.
Resultater/diskusjon: Sykepleiers holdninger, erfaringer og kunnskaper påvirker hennes vurderinger. Det anbefales at oksygenmetning, PaO2, puls, Hb og oksyhemoglobinets dissosiasjonskurven vurderes for å sikre faglig forsvarlig praksis. Nyfødtsykepleier må ha kunnskap om korrekt bruk av målemetoder og mulige feilkilder. Både overvåkingsutstyr og barnet skal vurderes før nyfødtsykepleier endrer oksygenfraksjon i inspirasjonsluften.
Konklusjon: Pulsoksymeter, transcutanmåler og blodgassmåling må brukes for å kontinuerlig overvåke barnets oksygenering. Tette alarmgrenser bør stilles inn for å bedre oppnå ønsket metningsverdi. En sykepleier per barn og bruk av retningslinjer kan forebygge hypoksi og hyperoksi ved administrering av oksygen.
